William Dozier's WONDER WOMAN Pilot
Starring Ellie Wood Walker... huh? From 1967, and brought to my attention by the PCL Linkdump. The Wonder Woman transformation at about 3:20 into the video is quite atrocious...
If you don't know who William Dozier is - listen to the narrator who comes in at the 3:20 mark and you should recognize the voice.
